About The Last Mile

The Last Mile (TLM) is a Bay Area-based nonprofit whose mission is to provide opportunities for personal and professional growth for justice-impacted individuals through in-prison education and technology training, while also offering a scalable model for change within a historically difficult-to-impact prison system. With programs operating in 9 states and growing, TLM prepares students for high-demand careers and supports them post-release through wraparound reentry services.

Position Overview

The Housing and Reentry Case Manager will lead the day-to-day implementation of the housing component of The Last Mile’s Adult Reentry Grant (ARG) funded by California’s Board of State and Community Corrections (BSCC). While this position will eventually support other reentry efforts, for the first two years the core focus will be overseeing all case management responsibilities tied to the ARG — including participant support, compliance reporting, and coordination with our external evaluation partner.

This is a Los Angeles-based role that will work remotely, with frequent in-person meetings and partner engagement in the region.

Key Responsibilities

Case Management (Primary Function)

  • Serve as the primary point of contact for justice-impacted participants placed through the Adult Reentry Grant in Los Angeles.

  • Provide individualized case management to 14–18 participants annually (approx. 50 over three years), including goal setting, progress monitoring, and service navigation.

  • Maintain accurate, timely case notes and documentation in alignment with grant requirements.

  • Work collaboratively with participants to support long-term housing stability, employment goals, and successful community reintegration.

  • Coordinate with The Last Mile’s internal Reentry Team to align services and referrals.

Grant Management & Compliance

  • Lead all case-related reporting and documentation required by BSCC, ensuring full compliance with ARG requirements.

  • Support the collection and submission of required data points including service delivery, housing placements, participant outcomes, and follow-up reports.

  • Provide regular narrative and quantitative updates to The Last Mile’s internal grants team and BSCC, with a focus on transparency and accountability.

  • Partner with the external evaluation firm to align data tracking, feedback loops, and outcome measurement strategies.

  • Identify potential risks or gaps in service delivery and proactively flag them to program leadership.

Community & Partner Collaboration

  • Build and maintain working relationships with housing providers, transitional housing programs, and reentry service organizations in Los Angeles.

  • Participate in regional coalitions and networking opportunities to strengthen referral pipelines and broaden service offerings.

  • Represent The Last Mile at partner meetings, site visits, and community events as needed.

What We’re Looking For

  • Minimum of 3–5 years of professional case management experience serving justice-impacted or vulnerable populations.

  • Demonstrated experience managing or supporting state or federal grants, including outcome tracking, reporting, and compliance.

  • Strong organizational and project management skills; ability to prioritize competing deadlines while maintaining attention to detail.

  • Familiarity with or willingness to learn BSCC expectations, reporting platforms, and grant evaluation processes.

  • Experience building partnerships with external agencies, including service providers and housing resources.

  • Excellent interpersonal and communication skills, including empathy, cultural humility, and the ability to build rapport with justice-impacted individuals.

  • Comfort working independently in a remote environment, while maintaining strong communication with a distributed team.

  • Lived experience with the justice system is a plus, but not required.

Preferred Qualifications

  • Bachelor’s degree in social work, public administration, criminal justice, or a related field.

  • Knowledge of housing navigation systems, including rental assistance, emergency housing, and public benefit programs (not required but helpful).

  • Previous experience supporting evaluation or research efforts related to public funding.

  • Familiarity with Los Angeles’ reentry and housing landscape.
